The following information does not cover situations such as commercial driver license, motorcycle, chauffeur, or restricted licenses. Please call the treasurer's office if you have questions.
Generally, when a student, under the age of 18, applies for a permit or graduated driver's license, the state requires that the student provide his certified birth certificate, social security card, and a parent/legal guardian affidavit (form 430018; Parent Written Consent to Issue Privilege to Drive or Affidavit to Obtain Duplicate).
Driver's license personnel will make the final decision on what is acceptable proof of identity. If you have questions concerning this requirement, please contact your local examining personnel for more information.
Generally, when an individual moves into Iowa and applies for an Iowa driver's license, the state requires that the individual provide his out-of-state driver's license, social security card, and proof of residency. The individual must be a resident of Iowa for at least 30 days prior to applying for an Iowa DL.

If your address changes, you must report the address change to DOT within 30 days. Any driver's license station can make an address change on your license for $1. Your license will always show your residence address, but your mailing address will also be requested.
NAME CHANGE:
Individuals wishing to change their name must provide acceptable proof for any name changes. Acceptable items are:
- Original or certified court-ordered name change containing full name, date of birth and court seal.
- Divorce Decree.
- Marriage license or certificate.
